چکیده

a 49-year-old man presented with a history of multiple sclerosis, causing him increased tone and spasticity in the lower extremities. this severely affected his ambulation and mobility. Following the titration of oral baclofen and tizanidine to maximum tolerated doses with little impact on his spasticity, the patient agreed to a trial dose of 50 μg of intrathecal baclofen (itB), which was effective in reducing his spasticity and improving mobility. He subsequently underwent implantation of a SynchroMed ii (Medtronic, Minneapolis, Minn) itB pump, with the tip of the catheter placed at the t10/t11 junction, and the pump placed in the right upper quadrant of his abdomen. the patient’s spasticity and mobility improved on a small daily dose of itB, 500 micrograms per ml (91 μg/day). He lived at a distance from the center, and appeared to be doing well with the small dose, and therefore had not had any further adjustments. Six months postoperatively, he noticed return of the spasms. His Twisted Catheter Causing Baclofen Pump Malfunction: A Case Report

Bladder stones – red herring for resurgence of spasticity in a spinal cord injury patient with implantation of Medtronic Synchromed pump for intrathecal delivery of baclofen – a case report

BACKGROUND Increased spasms in spinal cord injury (SCI) patients, whose spasticity was previously well controlled with intrathecal baclofen therapy, are due to (in order of frequency) drug tolerance, increased stimulus, low reservoir volume, catheter malfunction, disease progression, human error, and pump mechanical failure. We present a SCI patient, in whom bladder calculi acted as red herring...

Retraction of an intrathecal baclofen infusion catheter following suprapubic cystotomy: a case report.

INTRODUCTION Intrathecal baclofen, administered via a Baclofen pump, is used for patients with spasticity. We report here a case of intrathecal catheter retraction following surgery. CASE REPORT A male patient with adrenoleukodystrophy and a baclofen pump implant was admitted to the urology department with bladder stones. A transurethral cystolithotripsy and a suprapubic cystotomy were perfor...

How should an infected perinephric haematoma be drained in a tetraplegic patient with baclofen pump implanted in the abdominal wall? – A case report

BACKGROUND We present a case to illustrate controversies in percutaneous drainage of infected, perinephric haematoma in a tetraplegic patient, who had implantation of baclofen pump in anterior abdominal wall on the same side as perinephric haematoma. CASE PRESENTATION A 56-year-old male with C-4 tetraplegia had undergone implantation of programmable pump in the anterior abdominal wall for int...

Imaging evaluation of intrathecal baclofen pump-catheter systems.

ITB pumps are widely used in the treatment of intractable spasticity for many clinical indications, including cerebral palsy and spinal cord injury. High-dose intrathecal administration places the patient at significant risk for withdrawal in the event of device malfunction, necessitating rapid and complete evaluation of the pump-catheter system. This article reviews the approach to imaging eva...

False positive radiographical evidence of pump catheter migration into the spinal cord.

Intrathecal drug delivery systems are becoming an increasingly common modality used by physicians to treat patients. Specifically, chronic spasticity secondary to multiple sclerosis (MS) may be treated with intrathecal baclofen (ITB) therapy when oral antispasmodics do not provide adequate relief. ITB therapy is effective, localizes drug delivery, and does not have the same degree of intolerabl...
